Liability Waiver & Assumption of Risk Agreement

Effective Date: 2/28/2026

By participating in classes, programs, workshops, events, or services at The Sculpt Me Studio (“Studio”), you agree to the following:

1. Acknowledgment of Risk

I understand that participation in Pilates, fitness training, infrared heat exposure, and related physical activities involves inherent risks, including but not limited to:

• Muscle strain

• Sprains

• Falls

• Heat-related illness

• Aggravation of pre-existing conditions

• Serious injury

I voluntarily assume all risks associated with participation.

2. Medical Responsibility

I affirm that:

• I am physically capable of participating in fitness activities.

• I have consulted a medical professional if necessary.

• I will inform instructors of any injuries, pregnancy, medical conditions, or limitations prior to class.

I understand that The Sculpt Me Studio is not a medical provider and does not provide medical advice.

3. Prenatal & Postnatal Participation

If I am pregnant or recently postpartum, I acknowledge that:

• Participation is voluntary.

• I have received medical clearance where appropriate.

• I accept full responsibility for my participation.

4. Infrared & Heat Exposure

I understand that infrared therapy and heated environments may increase body temperature and heart rate.

I agree to:

• Stay hydrated.

• Leave class if I feel unwell.

• Inform staff of any adverse symptoms.

I assume all risks related to heat exposure.

5. Release of Liability

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless:

The Sculpt Me Studio, its owners, instructors, employees, contractors, and agents

from any and all claims, demands, damages, or causes of action arising from:

• Injury

• Illness

• Property damage

• Loss

whether caused by negligence or otherwise, to the fullest extent permitted by law.

6. Indemnification

I agree to indemnify and hold harmless The Sculpt Me Studio from any claims resulting from my participation.

7. Personal Property

The Studio is not responsible for lost, stolen, or damaged personal belongings.

8. Photo & Media Release

I grant permission for The Sculpt Me Studio to use photographs or video taken during classes for marketing purposes, unless I submit written notice declining.

9. Agreement & Electronic Consent

By signing electronically or participating in services, I confirm that:

• I have read and understood this waiver.

• I voluntarily agree to its terms.

• I understand I am giving up certain legal rights.

10. Age Requirement

• Participation in classes and services at The Sculpt Me Studio is limited to individuals 18 years of age or older.

• By signing this agreement, you confirm that you are at least 18 years old.

• The Studio does not permit minors to participate in classes or services.
